Managing your health with Halaxy

You should have received the email invite to create a Halaxy account - if you haven't received it yet, please contact your practitioner directly.

With the invite, you can create a Halaxy account and log in to:

Manage your appointments and clinical records
Communicate with your care network
Keep track of your invoices and payments
Track your health's progress over time

How to use your
Halaxy Personal Health Record

Directory & Online Bookings

Use [Halaxy's Directory](../search) to search for a practitioner or book an appointment with your care network at any time using your Halaxy Personal Health Record account. To book an appointment using your Halaxy account, follow the steps below:

1. Click __My Practitioners__ 2. Select __Make a booking__ 3. Complete the bookings 4. Track your upcoming appointment

Intake Form

Your practitioner can use Halaxy to share an intake form with you before the appointment, saving you time during the consultation. To use Halaxy's intake form to complete information online before an appointment, follow the steps below:

1. Click __My Health__ 2. Select __Notes__ 3. Select the appropriate intake form and complete if requested

Clinical Tools

Using Halaxy, your practitioner can share medical records and clinical assessments for you to complete remotely as part of your treatment. To review your clinical information and respond to questionnaires and any other notes sent by your practitioner, follow the steps below:

1. Click __My Health__ 2. Select __Notes__ 3. Select the appropriate clinical note to review and complete if requested

Payments

Halaxy allows you to manage your appointment invoices and payments via the platform. To simplify your bookings, you can securely store your credit cards details against your account by following the steps below:

1. Click __Profile__ 2. Select __Payments__ 3. Add __Credit Card__
